Congratulations on becoming a NIDA graduate. We are committed to continually improving the quality of our courses and services. To help us do this, we are keen to hear your opinion on your experience at NIDA, now that you have completed your course.
The survey will take about 10 minutes and we would really appreciate it if you could respond by the middle of May 2011. You have the option of completing the survey anonymously; however, should you wish to be entered into a draw to receive one of three $100 vouchers redeemable at Westfield shopping malls, you will need to respond with your name. Please note, your answers will be treated in confidence and the results of the survey will be combined into a single report for each course. This means it will not be possible to identify the source of individual comments.
